Seeking a Licensed Clinical Social Worker (LCSW)
Responsibilities
- Provide discharge planning and psychosocial interventions through assessment, advocacy, referral, and interdisciplinary collaboration.
- Prepare social histories and assess patients' social, emotional, and financial needs.
- Manage policies and procedures for setting and evaluating residents' long- and short-term goals.
- Assist healthcare providers in understanding social and emotional factors affecting patients.
- Document services in compliance with regulations and payer guidelines.
Qualifications & Skills
- Minimum 2 years of relevant work experience.
- Master’s degree from an accredited School of Social Work preferred.
- Licensed as an LCSW (must have independent decision-making skills).
- Excellent communication skills for interaction with staff, physicians, patients, and families.
- Knowledge of procedures, patient safety, and compassionate approach.
Key Duties
- Conduct psychosocial assessments and develop care plans.
- Cookordination with healthcare teams to support patient needs.
- Ensure compliance with healthcare regulations and standards.
