Supplemental Health Care is working with a Hospice Care facility in Lima, Ohio to hire Social Workers. Responsibilities

• Provide social work services to patients and families in a hospice setting
• Assess patient and family needs and develop appropriate care plans
• Coordinate with interdisciplinary team members to ensure comprehensive care
• Offer emotional support and counseling to patients and families
• Document all interactions and care provided in accordance with regulatory standards

Requirements

• Active Social Worker Licensure/Certification in Ohio
• American Heart Association BLS
• At least 1-year of experience as a Social Worker
• Prior experience in hospice care

$1,760 - $1,946 per week*
