Travel Hospice RN - $2,445 per week

Hospice Nurse, Hospice Multiple Locations. This position involves providing comprehensive home hospice nursing care, including patient assessments, developing care plans, and ensuring appropriate utilization of services. The role demands collaboration with interdisciplinary teams to deliver quality end-of-life care aligned with hospice regulations and insurance reimbursement guidelines.

Responsibilities

• Performs patient assessments and develops individualized care plans
• Ensures appropriate utilization of hospice services and maintains documentation within a healthcare environment
• Manages medication reconciliation and safety protocols
• Participates in interdisciplinary team meetings every two weeks
• Provides mentorship to new staff
• Provides orientation and cross-training for staff
• Participates in performance improvement activities and supports agency growth initiatives
• Demonstrates knowledge of hospice regulations, insurance reimbursement, and safe medication management

Requirements

• Current New Jersey RN License
• 2–3 years of RN experience in a home hospice environment
• Experience using an electronic medical record system
• Excellent critical thinking and communication skills
• Valid New Jersey driver’s license
• BLS certification

Schedule

• Days, 8-hour shift
• 37.5 hours per week
• Every other weekend with compensatory days during the week
