About the role
We are seeking an experienced Physical Therapist Assistant (PTA) to provide high-quality home health rehabilitation services under the supervision of a licensed Physical Therapist. The PTA will deliver prescribed treatment plans, document patient progress, and help patients improve mobility, strength, and independence in the home setting.
Coverage area includes Atlanta, Decatur, Clarkston, Scottdale, and may extend to other territories within a 60-mile radius of the facility as needed. Average productivity: 6–7 patient visits per day, depending on visit type.
Responsibilities
- Deliver physical therapy treatments according to the established plan of care.
- Assist patients in improving mobility, balance, strength, and functional independence.
- Document all patient visits accurately using Homecare Homebase (HCHB).
- Monitor patient progress and communicate changes to the supervising Physical Therapist.
- Educate patients and caregivers on exercises, mobility techniques, and home safety.
- Maintain timely and accurate clinical documentation.
- Travel to patient homes within the assigned coverage area.
- Collaborate with the interdisciplinary healthcare team to ensure quality patient care.
Requirements
- Active Physical Therapist Assistant (PTA) license.
- Home Health experience is required (no exceptions).
- Homecare Homebase (HCHB) documentation experience preferred. Candidates without HCHB experience may be considered at a reduced rate during a two-week training period, subject to manager approval.
- Current CPR/BLS certification.
- Valid driver’s license, reliable transportation, and current auto insurance.
