Details
Standard working hours are 8:00 AM to 5:00 PM
Expectation of at least 20 units of in-home patient care per week
Visit types include routine PT visits, start of care, evaluations, reassessments, supervisory visits, OASIS discharge and recertification visits, and more
Mileage reimbursement is provided at $0.70/mile starting from the closest branch location or first patient's home
Requirements
- Minimum 1 year of physical therapy experience required
- BLS certification required (AHA only)
- Valid state PT license required if applicable
- Ability to lift and carry objects weighing up to 50 lbs and perform frequent walking, standing, bending, and driving
- Full range of motion and ability to perform strenuous movements required for patient care
- Strong independent time management and self-motivation skills required
- Effective communication skills with a diverse patient population required
- Knowledge of state therapy laws and home health regulations (Medicare Conditions of Participation) preferred

Responsibilities include providing PT services per physician orders, conducting evaluations, reassessments, and OASIS documentation across all visit types
Works closely with patients, family members, physicians, and agency personnel on a daily basis
Expected to function independently and manage a personal caseload across multiple counties
Serves as a clinical education resource for students and volunteers and may assist with performance improvement initiatives
Must demonstrate knowledge of community resources and agency referral systems
RTO
Limited and may not extend more than 2 consecutive days at a time
Orientation is limited to no more than 2 weeks
