Position Highlights

- May work weekends if agreed upon with therapist and patient
- No on-call required
- Counties Covered: Prince George’s County and Montgomery County
- Upper Marlboro, Maryland
Responsibilities
- Perform comprehensive physical therapy evaluations and reassessments.
- Develop individualized treatment plans based on patient needs and physician orders.
- Treat orthopedic, neurological, post-surgical, cardiopulmonary, and medically complex patients.
- Provide gait training, balance training, therapeutic exercise, transfer training, and fall prevention education.
- Educate patients and caregivers on home exercise programs, safety strategies, and equipment use.
- Collaborate with nurses, physicians, case managers, and interdisciplinary care teams.
- Complete timely documentation and maintain home health compliance standards.
- Manage a home health caseload while coordinating visits based on patient needs.
Requirements
- Maryland PT License Required
- BLS Required
- Home Health experience preferred
