Travel Home Health Physical Therapist - $3,006 per week
Job Description
Ardor Health Solutions is seeking a travel Home Health Physical Therapist for a 13-week contract in Roseburg, Oregon.
Responsibilities
- Perform comprehensive physical therapy evaluations and reassessments.
- Develop individualized treatment plans based on patient needs and physician orders.
- Treat patients with orthopedic, neurological, post-surgical, cardiopulmonary, and medically complex conditions.
- Provide gait training, balance training, therapeutic exercise, transfer training, and fall prevention education.
- Educate patients and caregivers on home exercise programs, safety strategies, and equipment use.
- Collaborate with physicians, nurses, case managers, and the interdisciplinary care team.
- Complete timely documentation and maintain compliance with home health standards.
Requirements
- Active Oregon PT License
- BLS Certification
- Home Health experience preferred
- OASIS and HCHB experience preferred

Schedule
Full-time schedule of 40 hours per week, 8-hour shifts, days.
Pay
Mileage reimbursement of $0.50/mile.
