About the role
We are seeking a Travel Physical Therapist for a 13-week Home Health contract in Roseburg, Oregon. This opportunity offers the flexibility and independence of working in the home health setting while providing meaningful one-on-one patient care to adult and geriatric patients in their homes.

Responsibilities
- Perform comprehensive physical therapy evaluations and reassessments.
- Develop individualized treatment plans based on patient needs and physician orders.
- Treat patients with orthopedic, neurological, post-surgical, cardiopulmonary, and medically complex conditions.
- Provide gait training, balance training, therapeutic exercise, transfer training, and fall prevention education.
- Educate patients and caregivers on home exercise programs, safety strategies, and equipment use.
- Collaborate with physicians, nurses, case managers, and the interdisciplinary care team.
- Complete timely documentation and maintain compliance with home health standards.
Requirements
- Oregon PT License Required
- BLS Required
- Home Health experience preferred
- OASIS and HCHB experience preferred
