Travel Home Health Physical Therapist - $3,726 per week

Pride Health is seeking a travel Home Health Physical Therapist for an assignment in Johnstown, Pennsylvania.

About the Role

This is a travel position with a duration of 14 weeks, offering 40 hours per week on an 8-hour day shift. The role includes a weekly pay package of up to $3,800 gross (including a weekly non-taxable stipend) for candidates whose home location is more than 50 miles from Johnstown, PA. Mileage between patient visits will be reimbursed at $0.50 per mile.

Location: Johnstown, PA 15904. Coverage areas include Cambria, Northern Cambria, and Indiana County. The role guarantees 36 weekly hours with a productivity expectation of 30 points.

Shift schedule: Monday - Friday, 8:00 AM to 4:30 PM. No on-call or weekend requirements.

Responsibilities

• Conduct comprehensive home health physical therapy services, including Start of Care (SOC), initial evaluations, routine revisits, re-evaluations, and discharge assessments.
• Develop individualized treatment plans to improve patients' functional mobility while meeting productivity expectations.
• Deliver evidence-based therapy interventions and monitor/document patient progress accurately in Homecare Homebase (HCHB) and PointCare.
• Collaborate with physicians and the interdisciplinary team, educate patients and caregivers, and ensure compliance with Medicare, agency, and home health documentation standards.

Requirements

• Licensed Physical Therapist (PT) with a valid PT license.
• Experience with HomeCare HomeBase (HCHB) for documentation and PointCare.
• Prior home health experience.
