Responsibilities
- Provide one-on-one physical therapy in patients' homes
- Develop and implement individualized treatment plans
- Collaborate with an interdisciplinary team including nurses, therapists, and case managers
- Manage a territory-based caseload with flexible daily scheduling
- Treat diverse patient populations including orthopedic, neurologic, post-surgical, and medically complex patients
Schedule
- 40 hours per week, 8-hour day shifts
- 13-week contract
- Monday–Friday schedule
