Work Location: Mount Vernon, IL 62864Coverage Area: White, Hamilton, Franklin & Saline Counties, IL Weekly Gross Pay: Up to $2700.00 Schedule: Monday–Friday, 8:00 AM–4:30 PMHours: 36 hours/weekProductivity: 30 pointsOn-Call/Weekend: NoneMileage: Reimbursed between patient visitsPosition Summary:We are seeking a Physical Therapist (PT) to provide skilled physical therapy services to patients in their homes throughout the assigned coverage area.The PT will conduct evaluations, provide patient education, develop plans of care, and oversee the care provided by PTAs.Key Responsibilities:Perform home health physical therapy evaluations and assess patients' functional needs.Provide skilled PT services and education to patients and their families.Develop and oversee individualized physical therapy plans of care.Provide clinical direction and oversight for Physical Therapist Assistants (PTAs).Monitor patient progress and modify treatment plans as appropriate.Collaborate with physicians and the interdisciplinary home health team.Complete accurate and timely clinical documentation.Ensure care is provided in accordance with applicable home health standards and regulations.Required Qualifications:Current Illinois Physical Therapist (PT) license required.Strong clinical assessment and communication skills.Ability to provide care independently in patients' homes.Preferred QualificationsPrevious home health PT experience.Experience with Home Care Home Base (HCHB).Experience overseeing PTA plans of care.Dress Code: Any / No specific color requirement.Patient Care Setting: Home HealthClinical Specialty: Physical Therapist (PT)About PRIDE HealthPRIDE Health is the minority-owned healthcare recruitment division of Pride Global—an integrated human capital solutions and advisory firm.
