Responsibilities

• Perform comprehensive physical therapy evaluations and reassessments in patients' homes.
• Develop individualized treatment plans focused on improving mobility, strength, balance, and independence.
• Treat patients with orthopedic, neurological, post-surgical, cardiopulmonary, and medically complex conditions.
• Provide gait training, balance training, therapeutic exercise, transfer training, and fall prevention education.
• Collaborate with physicians, nurses, case managers, and interdisciplinary healthcare teams.
• Educate patients and caregivers on home exercise programs, safety strategies, and equipment use.
• Complete timely documentation while maintaining quality standards.

Requirements

• American Heart Association (AHA) Basic Life Support (BLS) Certification required.
• Arizona PT License required.
• Home Health experience preferred.
• OASIS experience preferred.
