Travel Home Health Physical Therapist - $2,646 per week

Job Description & Requirements

Supplemental Health Care is hiring a Home Health Physical Therapist for a 13-week contract starting ASAP in Ashland, Kentucky. The position requires an active, unrestricted Physical Therapist license, current BLS certification, 1+ year of recent home health experience, and familiarity with Medicare-certified home health guidelines and OASIS-E documentation.

• Active, unrestricted Physical Therapist (PT) license in the state of assignment
• Current BLS certification (AHA)
• 1+ year of recent home health experience (within the last 2 years)
• Familiarity with Medicare-certified home health guidelines and OASIS-E documentation
• Experience conducting PT evaluations and developing therapy plans
• Experience with EMRs such as Homecare Homebase, NetSmart, or Epic
• Cleared background check, driving record, and up-to-date health clearance (TB, immunizations)
• Reliable transportation with valid auto insurance and ability to travel between patient homes

Qualifications

Qualified candidates must have experience with gait and balance evaluations, home safety assessments, and mobility training. They should also be able to educate patients and caregivers on exercise programs, adaptive techniques, and safety strategies, and collaborate with RNs, OTs, SLPs, and physicians to ensure coordinated, interdisciplinary care.

Key Responsibilities

The Home Health Physical Therapist will perform initial and ongoing PT assessments and develop individualized plans of care. They will provide skilled interventions for strengthening, endurance, pain management, and fall prevention. Additionally, they will conduct gait and balance evaluations, home safety assessments, and mobility training. They will also educate patients and caregivers on exercise programs, adaptive techniques, and safety strategies, and collaborate with interdisciplinary teams to ensure coordinated care.
