Travel Home Health Physical Therapist - $2,646 per week

BrightSpring Health Services is seeking a Travel Physical Therapist for a 13-week Home Health contract in Grand Junction, Colorado. This position provides one-on-one care to adult and geriatric patients in their homes, focusing on mobility, independence, and confidence.

Responsibilities

• Perform comprehensive physical therapy evaluations and reassessments in patients' homes
• Develop individualized treatment plans to maximize mobility and functional independence
• Treat patients with orthopedic, neurological, post-surgical, and medically complex conditions
• Provide gait training, balance training, therapeutic exercise, transfer training, and home safety assessments
• Educate patients and caregivers on home exercise programs, fall prevention, and equipment use
• Collaborate with physicians, nurses, case managers, and other healthcare professionals
• Complete timely documentation and OASIS assessments as required

Requirements

• Colorado PT License Required
• Home Health experience required
• HCHB experience required
