Responsibilities
- Perform comprehensive physical therapy evaluations and reassessments in patients' homes.
- Develop individualized treatment plans focused on improving mobility, strength, balance, and independence.
- Treat patients with orthopedic, neurological, post-surgical, cardiopulmonary, and medically complex conditions.
- Provide gait training, balance training, therapeutic exercise, transfer training, and fall prevention education.
- Collaborate with physicians, nurses, case managers, and interdisciplinary healthcare teams.
- Educate patients and caregivers on home exercise programs, safety strategies, and equipment use.
- Complete timely documentation while maintaining quality standards.
Qualifications
- American Heart Association (AHA) Basic Life Support (BLS) Certification required.
- Arizona PT License required.
- Home Health experience preferred.
- OASIS experience preferred.
