Travel Home Health Physical Therapist - $2,625 per week

About the role

Reputable Home Health organization seeks temporary PT for Home Health in East Portland area. 30-mile radius and 25 patients on average per week using HCHB for documentation. This is a temporary need beginning in July.

Responsibilities

• Provide physical therapy services to patients in their home environment
• Document patient progress using HCHB software
• Adhere to HIPAA guidelines and maintain patient confidentiality
• Collaborate with interdisciplinary teams to develop and implement treatment plans
• Assess patient needs and adjust treatment plans accordingly
• Communicate effectively with patients, families, and other healthcare providers

Requirements

• Current state licensure as a Physical Therapist
• At least 1 year of clinical experience in a home health setting
• Bachelor’s degree in Physical Therapy
• Passing score on the NCLEX-RN exam

Qualifications

• Excellent communication skills
• Ability to work independently and as part of a team
• Strong organizational and time management skills
• Valid driver’s license and reliable transportation

Skills

• Proficient in Microsoft Office Suite
• Familiarity with HCHB software
• Knowledge of home health care protocols and standards

Pay

Pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with tax-free stipend amount to be determined.

Schedule

40 hours per week, 8 hour shift, starting date: 10/19/2026, duration: 13 weeks.
