A travel Home Health Physical Therapist is needed for a 13-week assignment starting August 24, 2026, in Jackson, California. The position requires 40 hours per week, 8-hour shifts.

Responsibilities

• Provide physical therapy services to patients in their home environment.
• Develop and implement individualized treatment plans based on patient needs.
• Document patient progress and outcomes in the EMR system (Matrix).

Requirements

• Current license to practice as a Physical Therapist in California.
• At least 1 year of experience in Home Health Physical Therapy.
• Proficiency in using the EMR system (Matrix).

Qualifications

• Passing score on the NCLEX-RN or equivalent.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage time effectively.

Pay

The pay package is based on 8-hour shifts and 40 hours per week (subject to confirmation), with a tax-free stipend amount to be determined. The estimated weekly payment includes hourly wages, meal & incidental expenses, and housing expenses.
