Home health setting serving a geriatric patient population (kupuna) in their homes on Maui
Patient population includes those with orthopedic, neurological, cardiopulmonary, and cognitive conditions, including post-op hip/knee/spinal surgery, stroke, Parkinson's, dementia, COPD, CHF, CAD, and PVD
Responsibilities span evaluation, OASIS completion, plan of care development, reassessments, and discharge planning
Productivity requirement of 25 units per week
Day shift schedule, 5x8-hour shifts (8:00–16:30)
Occasional floating to cover Oahu, Kauai, and/or the Big Island may be required; all float-related expenses are covered
Documentation via HCHB (Home Care Home Base)
Requirements
- Minimum 1 year of home health physical therapy experience required
- Current BLS certification (AHA) required
- Active Hawaii (HI) PT license required if applicable
- OASIS experience required; total months of start-of-care OASIS experience and frequency of Medicare starts of care per week must be provided for consideration
- Experience with geriatric, neurological, and orthopedic patient populations required
- Proficiency with adaptive and assistive devices, gait training, strength testing, pain assessment, and home exercise program development
- Strong time management and documentation skills required

Preferred skills include wound care, lymphedema management, multiple sclerosis, spinal cord injury, traumatic brain injury, and neurodevelopmental technique (NDT)
Scrubs in any color and closed-toed shoes are required
Position is based on Maui, home to Haleakala National Park, world-class beaches, and the iconic Road to Hana
Pay & Benefits
Pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with tax-free stipend amount to be determined
