Job Description

CenterWell Home Health is seeking an experienced Physical Therapist (PT) for a Home Health assignment in West Plains, Missouri. The PT will evaluate and treat patients in their homes, develop individualized care plans, and help patients improve mobility, function, safety, and independence.

Position Details

13-Week Contract
Monday-Friday, 8:00 AM – 5:00 PM
Estimated Gross Pay: ~$2,566/week
Taxable Rate: $33/hr
Lodging: $110/day
M&IE: $68/day

Requirements

Active MO PT License
Minimum 1 year of Home Health experience required
HCHB (Homecare Homebase) experience required
OASIS experience required
Reliable transportation
