Responsibilities

• Perform comprehensive physical therapy evaluations and reassessments.
• Develop individualized treatment plans based on physician orders and patient goals.
• Treat patients with orthopedic, neurological, post-surgical, cardiopulmonary, and medically complex conditions.
• Provide gait training, balance training, transfer training, therapeutic exercise, fall prevention, and functional mobility interventions.
• Educate patients and caregivers on home exercise programs, safety strategies, and proper equipment use.
• Complete timely documentation and maintain home health compliance standards.
• Manage a daily home health caseload, with skilled visits expected in the field.
• Collaborate with nurses, physicians, case managers, and interdisciplinary care teams.

Requirements

• Florida PT License Required
• BLS Required
• Home Health Experience Required
• OASIS Experience Preferred
• Documentation System: BayCare forms
• COVID vaccination or exemption required
• Flu shot or declination required
