Responsibilities
- Perform home health physical therapy evaluations
- Develop and implement individualized treatment plans
- Assess patient mobility, strength, balance, and functional limitations
- Provide therapeutic exercise and mobility training
- Educate patients and caregivers on safety and home exercise programs
- Career coordination with physicians, nursing, and interdisciplinary team members
- Complete timely documentation and OASIS assessments
Requirements
- Home Health Physical Therapist experience preferred
- OASIS experience preferred
- HCHB experience preferred
- Productivity expectation: approximately 30 points per week
- Monday-Friday schedule (8:00 AM-5:00 PM)
- Mileage reimbursement: $0.50 per mile
- Reliable vehicle required
- Current PT license required
- Valid driver's license and auto insurance required
