Travel Home Care Physical Therapist - $2,469 per week

Responsibilities

• Assess patients' conditions and develop individualized plans of care to maximize mobility and independence.
• Collaborate with an interdisciplinary team to ensure patients' needs are met effectively.
• Perform standard therapy measurement techniques, including range of motion, strength, and balance tests.
• Utilize various therapeutic modalities to provide optimal patient care.

Requirements

• Current Vermont Physical Therapist licensure.
• Home health experience and OASIS experience are mandatory.
• Proficiency with McKesson, Netsmart, or Homecare Homebase EMR systems is strongly preferred.

Pay

The Pay package is based on 8 hour shifts and 40.0 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
