About the role
CoreMedical Group is seeking a travel Home Health Physical Therapist for a 13-week assignment starting July 13, 2026, in Fort Wayne, Indiana.
Responsibilities
- Provide exceptional patient care in a home health setting
- Work 40 hours per week, 8-hour shifts
Requirements
- Current licensure to practice physical therapy in the state of Indiana
- Experience in home health settings preferred
- Passionate about providing high-quality patient care
Qualifications
- Master’s degree in Physical Therapy from an accredited program
- Current CPR certification

Pay
