About the role
American Medical Staffing is seeking a travel Home Health Physical Therapist for a 13-week assignment in Camden, South Carolina. This position offers 40 hours per week, 8-hour shifts, and a pay range of $2105.60 - 2210.88/week.
Responsibilities
- Deliver one-on-one, high-touch care to patients in their homes
- Conduct skilled nursing visits and assessments
- Create and manage individualized care plans
- Collaborate with families and interdisciplinary providers to improve patient outcomes
- Provide personalized physical therapy (PT) services to patients in their homes
- Assess patient mobility and functional status
- Develop tailored treatment plans that promote recovery and independence
- Utilize industry keywords such as patient-centered care, assessments, and therapy planning in your clinical approach
Requirements
- 2+ years of experience as a PT
- Valid Physical Therapist license
- Willingness to obtain South Carolina licensure
Qualifications
Qualified candidates must have at least 2 years of experience in Home Health.
Skills
Knowledge of patient-centered care, assessments, and therapy planning is essential.
