This position involves a 13-week traveling contract primarily focusing on Skilled Nursing Facility (SNF) and Long-Term Care (LTC), alongside some outpatient (OP), home health (HH), and assisted living (ALF/ILF) responsibilities. Responsibilities Provide high-quality physical therapy services to patients in various settings. Develop and implement individualized treatment plans. Monitor patient progress and adjust treatments as necessary. Collaborate with interdisciplinary teams to optimize patient outcomes. Educate patients and families on treatment plans and rehabilitation techniques. Qualifications Current license to practice as a Physical Therapist. Strong communication and interpersonal skills. Ability to work effectively within a team. Commitment to patient-centered care and rehabilitation excellence. Flexibility to cover various settings and patient needs as required. Work Schedule This position requires availability for Monday to Friday shifts from 8:00 AM to 4:30 PM, with a 30-minute unpaid break. Holiday coverage may be required.
