Travel Home Health Physical Therapist - $1,946 per week

Job Description & Requirements

Supplemental Health Care is seeking a Home Health Physical Therapist for a 13-week contract starting ASAP in Owensboro, Kentucky. The position requires an active, unrestricted Physical Therapist license, current BLS certification, 1+ year of recent home health experience, and familiarity with Medicare-certified home health guidelines and OASIS-E documentation.

• Active, unrestricted Physical Therapist (PT) license in the state of assignment
• Current BLS certification (AHA)
• 1+ year of recent home health experience (within the last 2 years)
• Familiarity with Medicare-certified home health guidelines and OASIS-E documentation
• Experience conducting PT evaluations and developing therapy plans
• Experience with EMRs such as Homecare Homebase, NetSmart, or Epic
• Cleared background check, driving record, and up-to-date health clearance (TB, immunizations)
• Reliable transportation with valid auto insurance and ability to travel between patient homes

Qualifications

Key Responsibilities

Key responsibilities include performing initial and ongoing PT assessments and developing individualized plans of care, providing skilled interventions for strengthening, endurance, pain management, and fall prevention, conducting gait and balance evaluations, home safety assessments, and mobility training, educating patients and caregivers on exercise programs, adaptive techniques, and safety strategies, collaborating with RNs, OTs, SLPs, and physicians to ensure coordinated, interdisciplinary care, and documenting all care in compliance with Medicare, OASIS-E, and agency standards.
