Responsibilities
- Deliver skilled occupational therapy services under the supervision of an OT, focusing on ADLs, mobility, and functional goals for SNF residents
- Implement and adjust treatment plans, monitor patient progress, and document outcomes in Rehab Optima (NetHealth) as part of routine care
- Absorb and assist residents with activities of daily living, therapeutic activities, and rehabilitation exercises; promote safety and independence
- Maintain accurate charting, equipment care, and adherence to infection control, safety, and HIPAA guidelines
- Collaborate with an interdisciplinary team (nurses, PT, SLP, social work) to support discharge planning and continuity of care
Requirements
- Active California license to practice as a Certified Occupational Therapy Assistant (COTA)
- Skilled Nursing Facility (SNF) experience
- Knowledge or experience with Rehab Optima (NetHealth) EHR system
- Ability to work Days, 38 hours per week, travel assignment
- Comfortable working in a SNF setting in Monterey, CA
