Responsibilities

• Complete the questionnaire thoroughly and provide all requested information.
• Indicate N/A for questions not applicable and provide explanations if necessary.
• Upload supplemental information sheets if additional details are needed.
• Upload certificates verifying the firm's status as per the requirements.
• Provide a list of judgments, arbitrations, liens, insurance claims, or lawsuits from the past 5 years.
• Answer questions regarding criminal background information, including convictions and charges.
• Provide details about insurance coverage limits and willingness to purchase additional insurance.
• Describe the firm's Health & Safety Program, including written programs, site-specific plans, inspections, investigations, and training.
• Detail the firm's safety measures such as fall protection, electrical safety, first aid, and emergency response.
• Respond to questions regarding OSHA training, safety meetings, and the use of fall-arrest equipment.
• Indicate whether the firm employs full-time competent tower climbers and rescue personnel and how often they conduct such activities.
