Transportation Coordinator

Responsibilities

• CookCare at Heritage is seeking a reliable Transportation Coordinator to join our team.
• Candidates with STNA or Direct Support Assistant (DSA) experience are strongly preferred.
• Career overview: This position is responsible for safely transporting residents to appointments while coordinating schedules with healthcare providers, nursing staff, and families.
• Coordinate resident transportation for medical appointments and approved outings.
• Safely transport residents using facility vehicles.
• Aid residents with entering and exiting vehicles as needed.
• Communicate appointment schedules with nursing staff, providers, families, and transportation vendors.
• Maintain transportation logs, mileage records, and required documentation.
• Complete routine vehicle inspections and report maintenance concerns.
• Keep facility vehicles clean, fueled, and ready for use.
• Provide courteous and professional service to residents and families.
• Assist with resident support duties as needed.

Qualifications

• STNA or Direct Support Assistant (DSA) experience preferred.
• Valid driver's license with a safe driving record.
• Ability to assist residents with mobility needs.
• Excellent communication and organizational skills.
• Dependable, compassionate, and professional.
• Healthcare or long-term care experience preferred.

Schedule

• Full-Time
• Monday-Friday
• Occasional weekends and holidays as needed
