Principal Duties And Responsibilities
- Provides assistance with receiving all incoming referrals and scheduling assessment visits
- Schedules and completes assigned client/family assessments to determine appropriateness for admission to Transitions program
- Identifies the needs of client/family and sets goals to meet those needs
- Implements and coordinates plan of services
- Facilitates on-going assessments/re-evaluations of client/family
- Appropriately refers clients/families to services in the community
- Documents visits and phone calls in data base to ensure accurate client/family records
Education
- Master's Degree in Social Work required
Experience
- Two to four years of experience working with patients/clients and families
- Experience working with hospice population preferred
Skills
- Skilled in fostering interpersonal relationships
- Skilled in oral and written communication
- Skilled in using computers
Licenses, Certifications
- LGSW or LCSW-C required
Physical Requirements
- Ability to life up to ten pounds maximum and occasionally lifting and/or carrying small item
- Working Conditions: Normal office work environment/field visits
Pay
Pay Range: $58,488.99 - $93,582.39
Final salary offer will be based on the candidate's qualifications, education, experience and alignment with our organizational needs.
