Transitional Support Specialist- Two Days

About the role

Provide transitional support to clients in need, assisting with daily activities and ensuring their safety and comfort.

Responsibilities

• Assist clients with basic needs such as meal preparation, personal hygiene, and medication management.
• Ensure clients are safe and comfortable at all times.
• Document client interactions and progress in a timely manner.
• Communicate regularly with clients' families and healthcare providers.

Requirements

• High school diploma or equivalent.
• Experience working with individuals who require support.
• Valid driver’s license and reliable transportation.
• Ability to work flexible hours, including evenings and weekends.

Qualifications

• Strong interpersonal skills.
• Good physical stamina and ability to lift up to 50 pounds.
• Pass a background check and drug screening.
