Transitional Housing RRH Case Manager

About the role

The Transitional Housing Case Manager secures and places survivors in transitional housing through the Transitional Housing Rapid Rehousing (TH RRH) program. The program provides housing assistance to survivors of domestic violence, dating violence, human trafficking, sexual assault, and stalking, with the goal of helping survivors regain empowerment and self-sufficiency through temporary housing or rental assistance. This role involves partnerships with landlords in Lake County, Illinois, and acting as the point person for VISPDAT assessments and client placements. The Case Manager reports to the Associate Director of Crisis Housing and collaborates with A Safe Place staff, community leaders, and centers of influence. The position is 24/7 on call.

Responsibilities

• Develop partnerships with landlords.
• Educate landlords about specific policies that must be followed to ensure client safety.
• Provide information on the Illinois Domestic Violence Act, Orders of Protection, Human Trafficking, and stalking laws to enhance client safety.
• Manage the application process, including required updates (e.g., income, household size) and re-certifications to help clients maintain stable housing.
• Oversee temporary housing for two families in units leased by A Safe Place.
• Perform outreach to community-based organizations, housing resources, and landlord associations to identify housing opportunities and build relationships.
• Coordinate intake assessments and eligibility evaluations to determine client needs and service appropriateness.
• Complete housing assessments, housing summaries, and VISPDAT evaluations.
• Create service plans and deliver in-house services such as employment skills training, education, economic assistance, therapeutic counseling, children’s services, and legal advocacy, as well as make referrals for additional services.
• Implement prescribed client assessment tools and comply with reporting requirements, including funder evaluations.
• Ensure documentation in client records is accurate, timely, and compliant with agency policies and funding requirements.
• Participate in a rotating on-call schedule (7 days) to support the Crisis Response team.
• Work one weekend every 4-6 weeks to support the HT House.
• Attend monthly coordinated entry meetings through the Lake County Homeless Coalition and additional required meetings.
• Collaborate with A Safe Place staff to refer clients to additional support services within and outside the organization.
• Support other staff and programs as needed.
• Collaborate with organizations that refer clients or provide services for A Safe Place’s clients.
• Recruit and develop relationships with community and corporate partners, including providing training as needed.
• Attend and participate in all team and staff meetings.
• Work as a team player within the organization.
• Conduct, attend, and participate in appropriate training and education as requested.
• Perform additional responsibilities as assigned.

Requirements

• Minimum: Bachelor’s degree in Social Work, Counseling, Education, or a closely related field.
• Ability to maintain a flexible schedule to meet client needs, respond to crises, and support the Crisis Response team.
• Bilingual preferred.
• Ability to motivate clients into action, engage tenants in training and education, and support their progress toward service plan goals and personal independence.
• Excellent written and oral communication skills, with proven ability to complete record documentation, develop group materials, and express information clearly.
• Excellent interpersonal skills demonstrating genuine concern and respect for clients and staff.
• Able to work independently and as part of a team, exercising mature judgment.
• Highly motivated self-starter with the ability to coordinate multiple projects and tasks.
• Knowledge of maintaining and executing confidential information.
• Ability to accept, understand, and relate sensitively to people of varied socio-economic, racial, cultural, and experiential backgrounds.
• Commitment to the philosophy and program of A Safe Place, including a dedication to survivor and children’s issues, and a belief in creating a society free from violence.
