Community Partners in Action (CPA) is one of the oldest organizations of its kind in the country, founded nearly 150 years ago to champion criminal justice reform and advocate for preserving the dignity of those in and out of prison. Through programs that include reentry and housing, youth initiatives, a nationally recognized Prison Arts Program, and holistic alternatives to incarceration, CPA provides a long-term impact that positively transforms individuals and society. Annually serving over 4,000 individuals throughout Connecticut, CPA is guided by a BEDI (Belonging, Equity, Diversity, and Inclusion) culture.

About the Program

The Hartford Transitional House (HTH) provides men with a supervised, structured, residential environment that is safe and supportive. HTH fosters a stable atmosphere to enhance residents' efforts and staff collaboration in effecting positive behavioral and decision-making changes. The program promotes responsibility, accountability, tolerance, respect, cooperation, and concern while meeting the needs of the referral source.

Responsibilities

  • Conduct resident intake, admission, and assessment (LSI-R) using motivational enhancement strategies.
  • Develop the Residential Individual Service Plan based on assessment outputs and provide recommendations for coordinating and sequencing program services.
  • Recognize and address resident ambivalence and resistance; help residents maintain motivation to change.
  • Collaborate with AIC, Adult Behavioral Health services, Advanced Supervision and Intervention Support, and other community-based services.
  • Coordinate and follow up on resident referrals to community-based services as necessary.
  • Monitor resident behaviors and implement the program behavior management system consistently.
  • Prepare accurate and timely progress and discharge reports.
  • Collaborate with referral sources and participate in weekly Service Team Meetings.
  • Supervise urine tests as required.
  • Administer initial PREA assessment within 24 hours of admission and PREA reassessment within 30 days.
  • Complete necessary case documentation for each resident in a timely manner.
  • Input resident data into the Contractor Data Collection System (CDCS) and maintain, update, and utilize the data as directed.
  • Utilize CPA's case management system (Securmanage) as directed.
  • Serve as a role model to clients and staff, modeling pro-social behaviors.
  • Promote a culture of respect, empathy, mutual support, belonging, and work to eliminate unconscious bias or discrimination.
  • Remain current on research and literature regarding effective strategies for changing offender behavior and reducing recidivism.
  • Attend and participate in all training and booster sessions related to the position.
  • Solicit resident satisfaction feedback.
  • Establish appropriate boundaries with residents and significant others.
  • Participate in quality assurance reviews of resident sessions and assessments.
  • Participate in related seminars, panels, and workshops to enhance professional development and agency visibility.
  • Perform all other duties as required.

Requirements

  • Bachelor's degree plus one year of experience in conducting assessments and case management duties (equivalent work experience considered in lieu of education).
  • Knowledge of criminal justice and court systems.
  • Ability to interact with people of diverse backgrounds.
  • Strong written and oral communication skills.
  • Experience in conducting assessments and case management functions preferred.
  • Strong computer skills.
  • Bilingual (English/Spanish) preferred.
  • Valid driver's license and reliable transportation.

Schedule

Includes two late days per week (12:30 PM – 8:00 PM).

Pay

$27.93 per hour.
