Transitional Care LPN- Adults and Pediatrics

About the role

BAYADA Home Health Care seeks to fill a SEAL team position in the Twin Cities. This team will assist with bringing home medically complex referrals, training and up coding nurses, and filling shifts for clients in need. SEAL team members will receive a higher rate of pay and enhanced benefits due to their flexible scheduling, traveling, and the acute level of care they can provide.

Responsibilities

• Provide adequate staffing up to 24/7 to meet various situations
• Support the area of biggest need by floating to support shifts
• Fill in staffing gaps to meet the needs of existing clients
• Assist with growing the field team by training new nurses

Requirements

• LPN with a current valid nursing license
• Homecare experience
• Commitment to agreed upon shifts
• Willingness to work in all assigned homes
• Willingness to train to the top of your license
• Willingness to precept onboarding nurses
• Commitment to attend school with clients if applicable

The pay for this role is commensurate with experience.

Qualifications

• Current valid nursing license
• Experience in home healthcare
• Ability to work flexible hours
• Willingness to travel
• Commitment to ongoing education and training
