Transitional Age Youth Case Manager

About the role

This is a hybrid position where the Case Manager will be located at Project Home Sites supporting transition-aged youth accessing Foster Youth to Independence Vouchers (FYI) through Project Home's FYI Program.

Responsibilities

• Serve as the central point of coordination for a network of providers working with each child.
• Coordinate and collaborate with providers to address home, school, medical, and community needs.
• Complete intakes for new clients, including psychosocial assessments and other intake assessments.
• Manage weekly documentation needs, make referrals, and complete initial and 90-day treatment plans.
• Provide a minimum of monthly contact with each family and continue collaborating with providers.

Requirements

• BA in a related field required.
• Strong organization and communication skills.
• Savvy computer skills to manage multiple systems.
• Current driver’s license and a clean driving record as outlined in Spurwink's Driving policy.
