About the Role

The STAR Kids Transition Coordinator plays a critical role in supporting individuals with disabilities as they transition from children’s to adult publicly funded services. This position ensures that youth aging out of the STAR Kids program are prepared for adult programs through early planning, coordination, and advocacy. The coordinator oversees all transition activities, from initial contact to final placement in STAR+PLUS, other HHSC programs, or community-based programs, while ensuring members live in their preferred integrated residential setting. Additionally, the role involves relocating members from nursing or intermediate care facilities back to community settings of their choice.

Responsibilities

• Assemble a transition team and develop a comprehensive transition plan to maximize independence and assist youth in qualifying for appropriate adult programs.
• Develop policies, informational materials, resource guides, transition assessments, and planning tools to support transition readiness and integration into adult services.
• Oversee and lead all transition activities from initial contact through final transition to STAR+PLUS or other community-based programs.
• Be knowledgeable about children’s issues, transition activities, local community resources, Medicaid eligibility, guardianship, and programs for individuals with complex needs.
• Conduct transition assessments resulting in a continuity of care plan and checklist to ensure no breaks in service or medication availability.
• Identify adult healthcare providers and assist in transitioning members to new providers.
• Work with nursing facilities (NF) or intermediate care facilities (ICF) to establish permanency plans for relocating members back to community settings.
• Establish communication links, monitoring schedules, written service plans, and post-relocation supports to ensure stabilization in community placements.
• Conduct other assessments as required by the STAR Kids contract.
• Negotiate with vendors for services or equipment as needed.
• Ensure appropriate referrals to agencies and assistance programs are completed.
• Complete thorough documentation of members’ plans of care, progress, outcomes, case reports, and status reports.
• Participate in the health plan’s performance improvement activities and committees as requested.
• Communicate with management regarding workload status and discrepancies.
• Ensure compliance with the STAR Kids contract and all service coordination requirements.
• Perform additional professional services as requested by leadership.

Requirements

• Bachelor’s degree from a four-year college or university, or equivalent combination of education and experience.
• Minimum of 3 years’ transition or community resource experience (pediatric experience preferred).
• 1 year of experience in managed care or case management.
• Previous experience with the State of Texas STAR+PLUS program or similar programs for SSI and disabled children preferred.
• Knowledge of Medicaid/Medicare and managed care criteria/guidelines.
• Working knowledge of various reimbursement mechanisms, including third-party requirements.
• Proficiency in Microsoft Office.
• Current driver’s license and insurance.

Skills

• Strong understanding of Texas Medicaid criteria and managed care contracts/guidelines.
• Knowledge of programs and services supporting the health and medical needs of disabled children.
• Commitment to maintaining confidentiality and compliance with regulatory agencies (e.g., Federal, State, URAC).
• Adherence to ethical business practices and corporate compliance guidelines.
• Ability to implement and follow health system and health plan policies for members with special healthcare needs.
