What Will You Be Doing
- Processes patient referrals and maintains client information in the database.
- Provides clerical and administrative support to DME Customer Service in day-to-day care management activities.
- Provides telephonic support for incoming and outgoing calls from patients, providers, Transition Coach and other team members related to the Transition Care program.
- Expands, updates, and maintains community resource database.
What Will You Come With
- 1-2 years of customer service support experience with the healthcare industry, Home Health Services preferred.
- Effective verbal and written communication skills.
- Ability to use various computer programs and applications, including IE EMR/EHR.
- Ability to self-motivate and work independently.
- Customer focus and adaptability to different personality types.
- Ability to multi-task, set priorities, and manage time effectively.
- Familiarity with Patient Referrals.
