What Will You Be Doing

  • Processes patient referrals and maintains client information in the database.
  • Provides clerical and administrative support to DME Customer Service in day-to-day care management activities.
  • Provides telephonic support for incoming and outgoing calls from patients, providers, Transition Coach and other team members related to the Transition Care program.
  • Expands, updates, and maintains community resource database.

What Will You Come With

  • 1-2 years of customer service support experience with the healthcare industry, Home Health Services preferred.
  • Effective verbal and written communication skills.
  • Ability to use various computer programs and applications, including IE EMR/EHR.
  • Ability to self-motivate and work independently.
  • Customer focus and adaptability to different personality types.
  • Ability to multi-task, set priorities, and manage time effectively.
  • Familiarity with Patient Referrals.
