What You’ll Need
- Bachelor’s degree in education, social work, or a related field
- At least one year of experience working with students with special needs in an educational setting
- At least one year of experience supporting career or transition services
- Knowledge of PA Department of Education regulations and transition planning requirements
- Strong communication, organization, and collaboration skills
What You’ll Do
- Develop and implement individualized career and transition plans aligned to IEP goals
- Career and transition services coordination
- Support vocational placements, transportation, and community-based experiences
- Teach and support students in skill development for work and adult life
- Collaborate with teachers, teams, families, and external partners to support transition outcomes
- Maintain accurate documentation, logs, and timelines related to transition services and IEPs
