Job Profile Summary
Hospital-wide weekend shift rotation required based on department operations. This role focuses on evaluating, developing, recommending, implementing, and carrying out policies and procedures related to the delivery of high-quality patient care. It also includes performing Care Navigation duties, supporting organizational processes, and providing administrative support.
Duties and Responsibilities
- Performs general office functions including typing, filing, email/mail correspondence, purchasing supplies, and maintaining confidentiality.
- Uses and maintains office equipment such as copy machines, fax machines, and computers.
- Greets visitors professionally and directs them to appropriate personnel.
- Supports assigned care managers by handling administrative and clinical tasks, assisting with special projects, and participating in care transitions.
- Assists with phone triage for Continuity of Care, establishing relationships with external contacts, and facilitating insurance appeals.
- Participates in a weekend coverage rotation to meet departmental needs.
- Enters documentation into the Electronic Medical Record (EMR) and documents critical information as requested.
- Communicates with patients and families regarding post-acute care planning and notifications.
- Prints daily census reports and serves as a liaison with various healthcare facilities.
- Faxes necessary paperwork and assists with copying and faxing of medical records.
- Rounds on inpatient units to deliver notifications required by CMS.
- Manages multiple tasks and prioritizes according to established criteria and protocols.
- Provides excellent interpersonal skills and remains calm under pressure.
Qualifications
- High School diploma or equivalent.
- One (1) year of related administrative experience.
Skills & Abilities
- Good organizational and communication skills, both verbal and written.
- Excellent computer skills using Microsoft Office.
- Ability to manage multiple tasks and prioritize accordingly.
- Knowledge of office management and daily operations.
- Ability to visualize the big picture independently with minimal supervision.
- Excellent proofreading skills.
- Resourcefulness and teamwork skills.
