Transition Assistant
About the role
Lifelong Access is seeking a Transition Assistant to support the High School Transition and Supported Employment Programs. This position provides direct support in various areas including programming, mini discovery, IEP meetings, on-the-job coaching, and job development activities. The Transition Assistant also assists with community presentations and networking events, collaborates with the Transition Coordinator to plan programming, and assists with transportation and supply organization.
Responsibilities
- Provide direct support in the areas of programming, mini discovery, and IEP meeting attendance, on-the-job coaching, and job development activities.
- Present information through community presentations/networking events.
- Collaborate with the Transition Coordinator to plan programming.
- Aid job seekers with developmental disabilities in attaining and maintaining a career path of their choosing.
- Order and organize supplies for Transition programming.
- Assist the Transition and Supported Employment teams with supports when needed.
Requirements
- Minimum of high school diploma or GED.
- At least one year of experience supporting people with disabilities.
- Familiarity with Microsoft Office applications (Word, Excel and Outlook).
- Work weekdays and early weeknight availability.
- Willing to travel to rural communities in McLean County.
- Must be at least 21 years of age.
- Valid Illinois driver’s license with safe driving record.
- Current proof of automobile insurance.
- Reliable transportation.
Physical Requirements
- Frequent sitting at a desk and working on a computer for prolonged periods.
- Frequent talking, hearing, and using hands, fingers to reach or feel.
- Frequent standing, sitting, stooping, bending, pushing, pulling, reaching, and walking for periods of time.
- Frequent use of hands to fingers, handle, or feel; and reaching with hands and arms.
