About the role

Coordinate patient transfers into and out of the LifePoint Health system through inbound and outbound calls. Facilitate the transfer process for patients moving to non-LifePoint facilities, ensuring timely and appropriate care.

Responsibilities

  • Perform initial admission screening and acceptance using approved clinical criteria to ensure consistency with current protocols and procedures.
  • Collaborate with multiple hospital departments and build relationships with facilities, physicians, and nursing staff.
  • Work closely with the Bed/Capacity Coordinator to identify, trend, and report barriers to patient throughput.
  • Support 24/7 operations alongside other Transfer Center Advocates.
  • Coordinate all components related to the admission, transfer, or consult of emergent, urgent, and elective patients from referring hospitals, clinics, urgent care clinics, and physician offices, ensuring timely and appropriate care and transport.
  • Process patient acuity systematically and make expert-level clinical decisions to facilitate patient flow through the continuum of care using electronic resources.
  • Document and prioritize patient information to provide an accurate picture of the patient’s condition for accepting physicians, nurses, or other healthcare workers.
  • Assign Transfer Priority Level for triage and transport to prioritize bed and transportation resources.
  • Maintain Transfer Center data collection and provide feedback to referring facilities on patient transition and outcomes.
  • Demonstrate knowledge of human growth and development, assessment, and care for patients of all ages.
  • Screen admissions for appropriateness of level of care, status, and bed type using approved criteria.
  • Apply Medical Necessity criteria competently.
  • Educate nurses, physicians, and ancillary staff on level of care and admission/transfer issues and opportunities.
  • Act as a liaison between physicians, patients/families, hospital staff, patient care areas, and outside agencies to promote efficient patient flow through professional communication.
  • Coordinate with the Bed/Capacity Coordinator for timely transfers, appropriate bed assignments, and accurate patient information.
  • Adhere to established protocols, procedures, and standards of care; escalate issues through the established Chain of Command in a timely manner.
  • Maintain current and accurate information in the Electronic Health Care Record.

Requirements

  • Graduate of an accredited school/college and holds a current RN license in state of residence.
  • Three years of related clinical experience in an acute care setting (ED or Critical Care preferred).
  • Ability to communicate effectively in oral, written, and electronic formats.
  • Must be able to multitask by attentively listening and simultaneously typing at 35+ WPM.
  • Must meet internet speed requirements: Upload 50+ Mbps, Download 100+ Mbps.

Pay

$29–36/hour.
