About the role

Performs moderately complex (journey-level) to advanced and/or supervisory (senior-level) case management work to develop and maintain long-term contact with assigned youth, their families, and providers for medical, social, educational, and related service needs. Work involves completing case file reviews, case planning, family involvement and group service delivery, developing goals, and evaluating outcomes. Performs discharge and release functions to ensure coordination of service delivery throughout the youth’s commitment. May require working in excess of 40 hours per work week, different shift patterns, and being on-call.

This position is within the approved TJJD Case Manager career path. An employee is hired at a level corresponding with education and experience requirements and may promote up to Case Manager IV without competing for a vacant position after meeting: (1) the education and experience requirements for the next higher level; and (2) the established performance criteria per agency policy.

Newly hired case managers or rehired case managers who separated more than 18 months will be required to attend the pre-service training academy. Case managers who return within 18 months of separation are not required to attend and will be placed in full-time status on their date of employment.

Levels of Work

  • Case Manager II: Performs moderately complex (journey-level) work under general supervision with moderate latitude for initiative and independent judgment.
  • Case Manager III: Performs highly complex (senior-level) work under limited supervision with considerable latitude for initiative and independent judgment.
  • Case Manager IV: Performs advanced and/or supervisory (senior-level) work under minimal supervision with extensive latitude for initiative and independent judgment.

Responsibilities

  • Performs stage assessments, requests missing documentation as needed, and conducts ongoing assessment for assigned caseload.
  • Completes Case Planning for assigned caseload, applies appropriate strategies to facilitate youth progress through Dialectical Behavior Therapy (DBT), and conducts risk assessments, including Behavior Chain Analysis (BCA), to initiate and promote change in behavior patterns. Proficiency in BCA is demonstrated by scoring a 3 or higher on independently submitted BCAs within 6 months of completion of OJT and initial caseload assignment.
  • Completes specialized referrals (e.g., additional assessments, Mental Health Treatment Program/Mentally Retarded Offender Program (MHTP/MROP) referrals, Texas Correctional Office on Offenders with Medical and Mental Impairments (TCOOMMI), etc.). Monitors caseload to complete transition planning.
  • Conducts and facilitates required groups for assigned caseload, including DBT groups. Plans for scheduled and unanticipated leave to ensure group services are provided by an approved alternate leader.
  • Participates in staff development, including Agency-approved treatment programs, group facilitation, Case Management Standards (CMS), and casework training.
  • Provides individual sessions to assigned caseload to conduct assessments, discuss identified behavior changes, and transition planning.
  • Provides updates to family/guardian about progress in the program, including stage assessments, behavior changes, and transition planning.
  • Completes documentation of services, stage assessment data entry, and maintains casework sub-file. Reviews and applies information found in youth records related to health, safety, and facility security; shares relevant information with staff.
  • Complies with policies, procedures, rules, and regulations; prepares and maintains records, forms, and reports.
  • Reports any presence or perception of racial, ethnic, and/or gender disparities.
  • Performs related duties as assigned and required travel up to 20% of the time.
  • Actively embodies and supports the Texas Model by demonstrating its core values in daily tasks and interactions.
  • Reports safety hazards and corrects them when possible. Completes required documentation for accidents/injuries within requested time frames. Maintains regular and predictable attendance in accordance with agency policies.

Requirements

Case Manager II - Pay Level One (1):

  • Unrelated Bachelor’s degree. No experience required.
  • OR High School diploma or equivalent, and:
    • Fifteen (15) semester hours of college coursework, and
    • One (1) year full-time wage-earning experience considering youth needs in a social, rehabilitative, or correctional environment.
