Territory Patient Admission Coordinator (Clerical)
Territory Patient Admissions Coordinator based in the St. Paul, MN office, onsite. Full-time position.
About the Role
The Territory Patient Admission Coordinator establishes and maintains communication with clients, family members, and referral sources within a designated territory. This role partners with agency staff, referral sources, and sales to achieve optimal patient satisfaction and outcomes. Responsibilities include the timely acceptance or decline of referrals based on predetermined agency criteria and facilitating the collection of all necessary information and documentation to complete a quality referral. The assigned territory may encompass one or more states and may change with business needs.
Responsibilities
- Collaborates and maintains effective communication with referral sources, sales teams, clients, and/or local agency operations to collect needed information for processing a quality referral.
- Documents all referral-related information, actions taken, and follow-up required in the medical record accurately and timely.
- Utilizes the agency’s software solution to facilitate the referral process by completing required workflow tasks within established time frames.
- Ensures timely attention to referrals received from various platforms (e.g., eFax, telephone, referral portal, or hospital EMR portal).
- Reviews eligibility alerts and responses for Medicare clients, where applicable.
- Requests H&P, discharge summaries, and other documents needed for ICD-10 coding by the CPOCC team if not received with the referral.
- Completes follow-up calls to clients, caregivers, or referral sources to collect or confirm information (e.g., out-of-pocket expenses, teachable caregiver details, missing demographic or physician information).
Requirements
- High school diploma or GED.
- Minimum of one year of experience in home care, hospice, or personal care services.
- Prior use of Electronic Medical Records (EMR), preferably HCHB.
- Experience dealing with a variety of healthcare payors (e.g., Medicare, Medicaid, commercial payers) preferred.

Pay
$23.00 to $30.00 per hour.
