Temp RN Medical Care Manager

About the role

A RN Medical Care Manager contributes to AHF by utilizing strong nursing clinical skills and understanding of the Ryan White Program within their specific area. With a caring and knowledgeable focus, you will work with our physician and other providers, the Care Management team, and the Healthcare Center team so that every Ryan White patient receives the support they require and access to available resources.

Responsibilities

• Work with physicians and other providers, the Care Management team, and the Healthcare Center team to ensure every Ryan White patient receives the support they require and access to available resources.
• Meet with patients and complete Needs Assessments, develop Care Plans, and provide ongoing education to clients so they understand their diagnoses and their medications.
• Collaborate with your team to meet all aspects of your patient’s health and psychosocial needs.
• Support patients in understanding their health status better to work towards achieving their healthcare goals.
• Consult with clinicians to develop treatment regimens that ensure optimal clinical outcomes, address treatment adherence education needs, and provide ongoing education for clients to self-manage their HIV by keeping healthcare appointments and taking prescribed medications.
• Conduct ongoing retention calls to ensure no gaps in retention efforts and to uphold cooperative relationships across divisions.

Requirements

Licenses and Certifications:

• Bachelor’s degree in Nursing (BSN), or License Practical Nurse (LPN) with a minimum of 1 year of Case Management experience.
• Knowledge of Ryan White Services is preferable.
