About the Role
The Intake Coordinator conducts intake interviews for clients inquiring about services to gather information, assess needs, and determine the nature and degree of problems. This role tracks and refers all intakes, ensuring clients are connected to appropriate services.
Responsibilities
- Screen clients with presenting problems.
- Conduct over-the-phone and in-home intakes, collecting basic information to begin a case file.
- Evaluate client histories and verify eligibility for services.
- Inform the Program Director of emergency and high-intensity intakes.
- Enter data into the VIVE system.
- Confirm start dates of services and schedule changes with vendors and clients.
- Follow up on home-delivered meals services with a 24-hour call and 15-day follow-up calls.
- Follow up on no-answer cases for home-delivered meals.
- Assist the case manager with 2-month monitoring calls.
- Provide accurate information on benefits and entitlements during intake.
- Counsel clients, advocate, and intervene on their behalf.
- Maintain complete and accurate case records.
- Compile statistics, prepare reports, and disseminate information as needed.
- Establish and maintain community outreach contacts.
- Perform other duties as required or assigned.
Requirements
- Bachelor’s Degree required.
- One year of related experience and/or training.
- Working knowledge of Microsoft Office Suite and other technology.
- Excellent communication and listening skills.
- Strong customer service skills with a focus on treating clients with respect and dignity.
- Able to multi-task and work independently with great attention to detail.
Working Conditions
- Business office environment with phone and computer use.
- Travel via public transportation may be required.
- This position may have the ability to work from home 2 times per week.
Pay
$25 - $27.47 per hour.
