Position Summary

Eventus WholeHealth delivers an integrated model of care to adults residing in various settings such as skilled nursing facilities, assisted living facilities, independent living, and personal homes. Services are provided through a network of Eventus WholeHealth providers including Physicians, Nurse Practitioners, Physician Assistants, Care Team Advocates, and in-house Support Staff. Telemedicine involves videoconferencing for patient medical information and pictures. Providers deliver medical services and consults with family, facility staff, and specialty providers as needed.

Essential Duties And Responsibilities

  • Provide primary-care and acute psychiatry services in accordance with Eventus site-specific protocols and the standard of care for family medicine, geriatric medicine, and internal medicine.
  • Perform other duties and activities as appropriate and/or assigned by Eventus Management to ensure quality or timely medical care and administrative responsibilities.
  • Manage facility and partner relationships, organize patient consents, CCM, and billing.
  • Triage needs and provide guidance, treatments, and care to residents of Eventus facilities.
  • Prioritize and manage multiple messages based on acuity.
  • Confirm treatment and telehealth consents are signed before telehealth visits.
  • Follow Eventus telehealth visit etiquette protocols.
  • Incorporate family members in the visit when appropriate.
  • Be familiar with telemedicine equipment, delivery platform, and software.
  • Complete documentation and sign all orders specific for the telehealth visit immediately after the visit.
  • Manage narcotic prescriptions according to Eventus RX Policy.
  • Notify patient's regular provider of visit and triage treatment plan.
  • Adhere to the latest triage schedules.
  • Communicate with facility staff and administration.
  • Provide verbal and/or written instructions or feedback regarding medications and other pertinent caregiver information.
  • Give time for questions from patients, families, or staff members.
  • Evaluate and establish when the patient should be seen next.
  • Document all required data on the same day.
  • Comply with Eventus standards, policies, and procedures.
  • Avoid gossip, negative comments, dual relationships, inappropriate attire, and breaches of confidentiality.
  • Maintain patient confidentiality, including appropriate use of communication tools.
  • Comply with HIPAA rules and regulations.
  • Adhere to state practice regulations as requested by Eventus.

Qualifications/Education

  • Master’s degree in Nursing from an accredited educational institution and a current and valid license to practice advanced practice nursing issued by the state where you are providing teletriage.
  • All certifications necessary to perform one’s responsibilities must be current and valid.
  • Specialization in geriatric, family, or internal medicine desired.
  • ALF or SNF experience desired.
  • OR have graduated from a physician assistant educational program accredited by the National Commission on Accreditation of Allied Education Programs and passed the national certification examination of the National Commission on Accreditation of Certification of Physician Assistants.
  • Licensed in one of the following states to start (NC, IN, KY, VA, and OH).
  • State licenses required within 4 weeks of hire.
