Role Overview
The Advanced Practice Clinician (APC) provides telehealth urgent care services remotely, delivering high-quality, cost-effective care across various settings. This part-time position involves managing acute, chronic, and behavioral health conditions, coordinating care transitions, and documenting findings in compliance with regulations. The APC collaborates with patients, families, and healthcare teams via telephonic and video platforms, ensuring seamless care delivery and patient satisfaction.
Responsibilities
- Provide telehealth assessments, treatment, and care management using EMR systems
- Coordinate care transitions and address member needs effectively
- Communicate professionally with patients, providers, and care teams
- Document clinical encounters accurately and adhere to quality standards
- Participate in quality initiatives and mandatory training
Qualifications & Skills
- Active, unrestricted NP license in required states
- Board certification through a recognized nursing board (e.g., AANP, ANCC)
- Proficiency in electronic medical records and telehealth platforms
- Minimum 3 years of clinical experience (preferred)
- Strong communication, adaptability, and customer service skills
