Telephonic Nurse Practitioner - Massachusetts & New York Licenses Required

Role Overview

The National On Call advanced practice clinician (APC) provides telehealth care, offering virtual assessments and management of patients' medical and behavioral health conditions in a remote environment. Responsibilities include delivering high-quality, cost-effective care, coordinating transitions across care settings, documenting thoroughly in EMR, and ensuring compliance with CMS regulations. The role involves managing acute, chronic, and complex cases, engaging in care planning, and participating in quality initiatives.

Key Duties

• Provide telephonic and video-based care to members, caregivers, and facility staff, ensuring patient privacy.
• Utilize EMR proficiently for documentation and care delivery.
• Manage medical and behavioral conditions collaboratively with physicians and specialists.
• Coordinate care transitions, review orders, and facilitate advanced care discussions.
• Ensure timely, compliant documentation and participate in ongoing training.
• Work flexibly across shifts, including nights, weekends, and holidays, with remote work options within the U.S.

Qualifications & Skills

• Master's or Doctorate in Nursing with active, unrestricted NP license; board certification (Family, Adult, Gerontology, or Acute Care Nurse Practitioner).
• DEA licensure or ability to obtain; proficient in electronic medical records.
• Preferred: 3+ years clinical experience, additional state licenses, geriatric or complex care experience, strong communication skills, adaptability, and ability to work independently.

Compensation

Compensation ranges from $109,500 to $164,000, including benefits, incentives, and 401k.
