Telemedicine & Behavioral Health Provider (NP/PA/PMHNP) - Skilled Nursing & Post Acute Care

About the role

Telos Health Systems is building a collaborative team of Nurse Practitioners, Physician Assistants, and Psychiatric-Mental Health Nurse Practitioners to provide high-quality telemedicine care to skilled nursing and post-acute patients throughout North Carolina. This role combines acute and routine medical care with behavioral health support, including encounters focused on common psychiatric, behavioral, and cognitive concerns. Dual certification in primary care and psychiatric-mental health is preferred. Providers care for a geriatric and post-acute population with varying degrees of medical, cognitive, and psychiatric complexity, supported by structured protocols, comprehensive onboarding, ongoing mentorship, and a team-oriented telemedicine environment. The position is ideal for clinicians who value clinical decision‑making, autonomy, and meaningful collaboration with bedside nursing teams while helping support safe treat‑in‑place care and appropriate escalation when necessary.
