About the role

The Team Manager leads the Patient Care Team, overseeing all patient care services including volunteer and bereavement support. Key responsibilities include:

  • Supervising, evaluating, and coordinating interdisciplinary team members.
  • Serving as patient advocate and coordinator for community health care providers.
  • Maintaining patient records and ensuring timely recertification decisions.
  • Ensuring appropriate and responsive team services, including staffing, medications, HME, supplies, and level of care changes.
  • Managing the team schedule and coordinating services 24/7.
  • Facilitating interdisciplinary meetings and updating the plan of care.
  • Participating in on-call rotations.

Responsibilities

Directs all patient care services, reviews admissions, collaborates on palliative care measures, and ensures accurate documentation.

  • Reviews pending and new admissions, assesses eligibility and appropriateness.
  • Collaborates with physicians on pain and symptom management.
  • Monitors team services and ensures appropriate staffing, medications, and care levels.
  • Manages team schedule and coordinates services 24/7.
  • Facilitates interdisciplinary meetings and updates the plan of care.
  • Facilitates on-call rotation.

Requirements

The ideal candidate must have:

  • Reliable transportation with appropriate license and insurance.
  • Private telephone in home.
  • Knowledge of primary medical and nursing care principles, with a working knowledge of oncology and palliative care.
  • Understanding of interdisciplinary health care team dynamics and the process of dying and bereavement.
  • Working knowledge of VITAS information system (Vx) and ability to utilize management reports.
  • Two years of supervisory experience or equivalent in a health care organization.

Qualifications

  • Bachelor's degree preferred.
  • Current and valid R.N. License to practice in the state where the VITAS program is located.

$100k-$110k, based on experience.

Schedule

Office-based, Monday-Friday, 8am to 5pm, with one field day required per week.
